Let's start a new referral
Below is an online version of the NHS Generic Referral Form that we have simplified in order to make referrals easier and swifter for GPs. Should you prefer to use the full version of the NHS Generic Referral Form, this can be downloaded here, and should be returned once complete to gp@problemshared.net. Please note that using the full version of the form will increase the time required to submit the referral and will result in longer processing time.
You will need either:
- Your patient’s self-assessment forms for ADHD or Autism (or both) or
- Be able to fill in your patient’s self-assessment forms on their behalf
After you submit the form your client will get an email inviting them to enrol.
